The SFVAHCS is comprised of many clinics to best serve you. You may opt to receive care on-site at one of these locations, or via telehealth.

Schedule a new patient visit: Call 415-750-2110 and request a new patient visit. Our New Patient Coordinator will work with you to schedule an appointment with a primary care provider of your choice.

Get ready for your appointment by:

  • Downloading the new patient welcome packet:
  • Completing the new patient intake form: New Patient Intake Form (PDF)
  • Completing the personal health inventory: Personal Health Inventory Form (PDF)
  • Preparing a complete list of your medications and supplements or bringing the medications
  • Gathering copies of prior medical records and vaccination history

Veteran Primary Health Care at the Medical Practice Clinic

Medical Practice Clinic is the name of the flagship primary care clinic atthe San Francisco VA Healthcare System (SFVAHCS) and is located at the San Francisco VA Medical Center (SFVAMC). The Medical Practice Clinic provides comprehensive primary care services on-site and via telehealth. See the “Medical Practice Clinic” page for more information. In addition to Medical Practice Clinic, we also have 8 clinics located throughout the SFVAHCS (see “Community Based Outpatient Clinics” page)

Community Based Outpatient Clinics

We have many clinics to best serve you. Besides the Medical Practice Clinic located at the San Francisco VA Medical Center, we have a number of Community Based Outpatient Clinics (CBOCs) that you may find more convenient. These clinics provide basic primary care and mental health services in addition to a widening array of other clinical and diagnostic services. All CBOCs have designated Women’s Health Primary Care Providers. Our CBOCs are located throughout the Bay Area and north coast and include Clearlake VA Clinic, Eureka VA Clinic, Oakland VA Clinic, San Bruno VA Clinic, San Francisco VA Clinic, Santa Rosa VA Clinic, North Santa Rosa VA Clinic, and the Ukiah VA Clinic.

Comprehensive Women’s Health Clinic

The Comprehensive Women’s Health Clinic offers gender-sensitive primary care, cervical cancer screening, reproductive health care, sexual health services, mental health care, and maternity care coordination in a separate, designated space for women and trans Veterans at the San Francisco VA Medical Center (SFVAMC). Additionally, all Community Based Outpatient Clinics (CBOCs) in the San Francisco VA Health Care System (SFVAHCS) have designated Women’s Health Primary Care Providers who can deliver high-quality gender-sensitive primary care to women Veterans. Additionally, we have Gynecology specialty services at the SFVAMC and Santa Rosa Clinic.

Infectious Disease/HIV Primary Care Clinic

We offer:

  • Testing and screening for hepatitis C, HIV/AIDS, tuberculosis, and other infectious diseases
  • Prevention strategies and treatment
  • Compassionate care and counseling for people who are living with infectious disease
  • HIV and Infectious Disease specialty services are available at the San Francisco VA Medical Center and the Oakland VA Clinic
